  • All payments must be made in Japan yen
  • All participants making a scientific presentation are to register in advance simultaneous to submitting their presentation.
    Presenters will only be charged half the normal registration fee.
  • Those registering as students, Post-doctor or Co-medical staff will be required to show identification at the on-site Registration Desk.
  • Program & Abstracts of the Symposium will be sent in advance to all participants completing registration by November 30, 2002

DEADLINE FOR ADVANCE REGISTRATION : November 30, 2002
After this date, registration application will be accepted on-site at the Symposium Venue only.

    Bank Account details will be forwarded by return E-mail acknowledgement.
  2. Credit Card (VISA, AMEX, MasterCard)
    Card number, expiration date, and name of card holder, exactly as they appear on the card, are required.

CONFIRMATION OF ADVANCE REGISTRATION

    Those who have completed on line Registration will receive a Registration Confirmation E-mail in return. If you do not recieve this E-mail within 24 hours after completing your procedure, please notify Registration Secretariat by e-mail. A letter of Confirmation & Receipt of Advance Registration will be forwarded to each registrant by mail after early October, 2002. This letter must be presented at the Registration Desk upon arrival.

ON SITE REGISTRATION

    An on-site Registration Desk will be in operation at the Symposium site. Please note that the deadline for advance registration is November 30, 2002

CANCELLATION AND REFUND POLICY

    In the case of a cancellation, the Secretariat must be notified in writing or by E-mail. An administrative charge of 5,000 yen will apply to all cancellations made before November 1, 2002. No refunds willbe made on cancellations made after this date.
